What does a Choice Care Membership include?
A one year membership in Choice Care costs $2000. Family and corporate rates are shown in the table below. The annual fee pays for all your care from Dr. Zwelling for twelve months.

The annual fee is paid directly to Choice Care and is not covered or reimbursed by your medical insurance. You will need to keep your health insurance because your Choice Care retainer covers only the services you receive from Dr. Zwelling. If you require hospitalization, lab tests, medical specialists or other medical services you will need a health plan to cover the costs.
Many of our patients have discovered that an HSA (Health Savings Account) is the perfect complement to concierge care and can offset a significant portion of the cost of joining Choice Care. Your financial advisor or insurance broker can help you decide if an HSA is right for you, and you can ask Dr. Zwelling for more information about how concierge medicine works with an HSA to keep you healthier and conserve your healthcare dollars.
You may join Choice Care at any time during the year. Membership is based on an annual contract payable at the start of the membership period. A quarterly payment schedule is available for a per-payment billing fee of $25, and corporations can elect to pay quarterly or monthly with no extra fees. Checks, cash and credit cards are accepted.
